Goals of care are discussions you and your Substitute Decision Maker (SDM) have with your health-care team to determine a care plan based on your current clinical situation.
Goals of care discussions revolve around your disease, prognosis, and treatment. Discussions can be different each time but will continue to reflect the values, beliefs and wishes you determined as part of your Advance Care Plan.
Discussions could be about starting or stopping treatment (i.e. dialysis) or whether or not to accept cardiopulmonary resuscitation (CPR).
