Nav-CARE

What we do

Nav-CARE is a free volunteer navigation program that supports people with declining health to live as well and independently at home for as long as possible. Specially trained Nav-CARE volunteers help clients to access resources and services in their community, while providing companionship and emotional support.

 

Nav-CARE volunteers can assist with many things including, but not limited to:

  • identifying resources and services available in the community;
  • assisting individuals with declining health to re-engage in activities and hobbies that are meaningful to them;
  • listening and providing support as individuals and families navigate important decisions about the types of care they would like to receive and where they would like to receive that care;
  • offering one-on-one relationships tailored to the needs of individuals and families facing declining health; and
  • helping to relieve feelings of isolation and loneliness often experienced by individuals with declining health.

Who we see

Clients who could benefit from the Nav-CARE program include individuals with declining health who are living at home or in a personal care home.

Our Team

The Nav-CARE team consists of specially trained Nav-CARE volunteers who are supported by a Nav-CARE coordinator. The Nav-CARE coordinator helps to match clients with a volunteer based on considerations such as the needs of the individual client/caregiver, client interests and geography.
