Palliative care in the community

What we do

We provide palliative and end-of-life care in the community both in private homes and personal care homes.  The type of services you receive are based on your care needs, and can include:

• home visits from member(s) of the interdisciplinary team;  
• support to help you manage pain and other symptoms;   
• access to equipment and other supplies needed for care, such as a hospital bed, incontinence supplies, nutritional supplements and oxygen;  
• access to home support for personal care, household management and respite, as applicable;  
• individual and family counselling;   and/or
• spiritual support .

Anyone diagnosed with a life-limiting illness or condition.

Please reach out to your health-care provider or call your local community health office if you or someone you know has a life-limiting illness and could benefit from palliative care in the community.
